Experts point out that the maternal mortality rate is 100 per 13,6 thousand live births in our country, according to the latest data, and draw attention to the fact that this rate is 100 per 11,0 thousand live births in developed high-income countries. Emphasizing that starting with pregnancy and progressing with a midwife in this journey, Dr. Faculty Member Tuğba Yılmaz Esencan said, “Thus, the conclusion of this journey with a healthy birth will undoubtedly be possible with the effect of midwifery follow-ups. The golden formula for reducing the risks during childbirth is regular pregnancy follow-ups and midwifery follow-ups. said.

Üsküdar University Faculty of Health Sciences Midwifery Department Faculty Member Tuğba Yılmaz Esencan made evaluations about the risks that occur during childbirth and the precautions to be taken.

Deviations and risks should be identified at an early stage

Stating that pregnancy and subsequent labor is a journey that takes place mostly in the physiological flow, Dr. Lecturer Tuğba Yılmaz Esencan noted that each birth is a new beginning and said, “For this reason, when we look at the essence of pregnancy and birth, we choose to focus on existence and the beauties that new energy will bring without choosing to think about the negativities and risks, predicting that everything will go well because it is physiological. A woman transformed by a growing miracle is on a unique journey. Deviations and complications that may be encountered in this journey, albeit a little, can also be a result of the process.” she said.

Risks can be avoided through precautions

Noting that the most important thing at this stage is to identify the deviations and risks from this mentioned path of approach, Dr. Lecturer Tuğba Yılmaz Esencan said, “Thus, with the precautions taken, we can prevent complications that may develop before the risks increase. But if we can't diagnose and take precautions early, these risks can cause the most important health problems that negatively affect the lives of women of reproductive age. At the same time, these health problems and maternal mortality rates show parallelism with the development indicators and reproductive health service quality of the country where they take place.” said.

Postpartum hemorrhage ranks first

Stating that according to the latest data in our country, the maternal mortality rate in 2018 was 100 per 13,6 thousand live births, Dr. Lecturer Tuğba Yılmaz Esencan said, “In countries that are included in the developed upper income group, this rate is 100 per 11,0 thousand live births. Considering the causes of maternal death, postpartum hemorrhage ranks first, although 70 percent of this is due to the risks and difficulties encountered during childbirth. In order for the labor to go smoothly, it is expected that the contractions during birth and the opening of the cervix and the thinning of the cervix should accompany each other in a harmony and in the targeted time. Any deviation in this area is defined as risky labor.” said.

Risks can be reduced with alternative interventions

Stating that risky labor also gives warning signs to the following midwife and obstetrician about minimizing the risks, Dr. Faculty Member Tuğba Yılmaz Esencan said, “At this stage, risks can be minimized by planning alternative interventions to protect the health of both mother and baby. For this reason, it is vital to monitor the signs of risky labor and to take precautions in the early period.” he said.

4P is effective in difficult birth

Dr. Lecturer Tuğba Yılmaz Esencan said that difficult labor or birth difficulty is used to name situations where labor deviates from the normal course of labor by definition and continued as follows:

“Dystocia, which is used synonymously with difficult labor, is also used to mean the pause of labor during labor, the opening of the cervix, the stopping of the baby's journey in the uterus, or the hesitation of both parameters. When we look at the causes of difficult birth, we come across four main factors that are effective in labor. These factors are used in the form of 4P, with the abbreviation of English words. While each factor may appear alone during the action, it can also occur together. Difficult labor occurs as a result of deviations from the path that occurs in these factors, which ensure that every stage of labor is harmoniously harmonized. Difficult labor is the most common indication for cesarean delivery.”

Dr. Instructor Member Tuğba Yılmaz Esencan shared the main factors used as 4Ps as follows:

  • Power: Forces in labor-Birth Waves
  • Passenger: Passenger- Birth Object-Fetus
  • Passageway: Birth Path- Bone Pelvis and Soft Tissues
  • Psyche: Psychological state – Woman's Mood

Follow-up until birth is of great importance

Reminding that a healthy life first begins in the womb, Dr. Faculty Member Tuğba Yılmaz Esencan said, “For this reason, close monitoring of the mother and baby during pregnancy is of great importance until the delivery. However, some unforeseen risks may be encountered in labor. These problems include bleeding, slowing or stopping of the birth waves or faster than expected birth waves, insufficient for birth, postural disorders of the baby in the mother's womb, disorder in the harmony of the baby's head with the mother's pelvis, the baby's head being larger than the mother's pelvis bone, the pelvic bone. There are physiological adaptations such as being narrow for birth, as well as the mother's not being ready for birth psychologically. It is very important for the woman to adapt not only physically but also mentally and mentally to the birth, and the absence of this adaptation also lays the groundwork for the problems that threaten the life of the mother and the baby at birth.” she said.

Progress with a midwife will ensure a healthy way

Emphasizing that starting with pregnancy and progressing with a midwife in this journey, Dr. Faculty Member Tuğba Yılmaz Esencan said, “Thus, the conclusion of this journey with a healthy birth will undoubtedly be possible with the effect of midwifery follow-ups. The golden formula for reducing the risks during childbirth is regular pregnancy follow-ups and midwifery follow-ups. In this way, risks can be detected early and prevented. Childbirth preparation training is very important in preparing pregnant women for birth. With these trainings, the pregnant woman can become the hero of her own birth.” used the phrases.

It is necessary to be prepared for difficulties

Üsküdar University Faculty of Health Sciences Midwifery Department Lecturer Tuğba Yılmaz Esencan stated that during the follow-up of the pregnant woman at the time of birth, a midwife accompanying her closely followed, and she concluded her words as follows:

“To cope with the difficulties encountered at this stage, it is necessary to be prepared first, to maintain calm and to identify the source of the problem. At this stage, the pregnant woman should relax, do breathing exercises, support her to cope with anxiety (important for the progression of labor and energy consumption), seek solutions by consulting a professional healthcare team and consult a gynecologist, especially during emergency problems encountered during delivery. Having a birthing team ready will be lifesaving. Close monitoring of important vital indicators such as blood pressure, pulse, respiration and body temperature of the pregnant, determination of the blood group of the pregnant and the readiness of blood preparations for a possible situation, follow-up in terms of infection, and in addition, monitoring and follow-up of the baby's heart sounds during the birth difficulties faced by the mother and the mother. These are important parameters for us to monitor the health of the baby and manage these problems.”
