Üsküdar University Health Services Vocational School (SHMYO) Home Patient Care Program Head Lecturer Büşra Ecem Kumru evaluated patient care at home and what needs to be done.

Lecturer Büşra Ecem Kumru said that home health services are offered through training and research hospitals operating under the Ministry of Health, home health service units established within general or branch hospitals, community health center, family health center.

Expressing that the classification of home care varies, Kumru said, “These classifications vary according to whether the service is medical or social service, according to the duration (short-term, medical-based care service or long-term, social services), and according to who provides the care provided. ” said.

Noting that many health services are included in the services offered within the scope of home care, Kumru listed these services as follows:

  • Home health service delivery: Nurse, doctor visit, home care technician
  • Support health services: Psychotherapy, physical therapy, podiatry, speech and occupational therapy
  • Personal care/ Self-care services: Dressing, feeding, washing
  • Housework service: Cleaning, shopping, security

Lecturer Büşra Ecem Kumru said that the home care process, which brings a burden for both the caregiver and the caregiver, will be positive for all parties in the care process, especially if it is carried out in professional hands and within a certain system at the national level.

Stating that home care allows patients the highest level of freedom they can achieve, Büşra Ecem Kumru said, “Home care personnel may be less stressful by indirectly being affected by the fact that they have a more special relationship with their patient and the patient who is not restrained in the hospital environment is less nervous. ” said.

Homecare aides do 80% of the application

Emphasizing that home care is a team effort, Kumru said, “Physicians, physiotherapists, psychologists, social workers, occupational therapists, speech therapists, nursing nurses, home care assistants are the natural parts of this team. Home care assistants are an increasingly important member of the team, they cover 70-80% of home care applications.

Expressing that home patient care is also applied on an international scale, Kumru said, “Florance Nightingale is the first school in Liverpool to train visiting nurses to provide home care services with a 1862-year training program in 1,5. According to the records of the American National Home Care Association, there were approximately 1995 home care companies and 15 healthcare professionals working in these companies in 700.” said.

Lecturer Kumru noted that the first regulation regarding the delivery of home care services in our country was the "Regulation on the Delivery of Home Care Services" dated 10.03.2005 and numbered 25751, which was issued for private home care services companies. It was issued in order to regulate the work and supervision of these institutions and organizations and the procedures and principles that these institutions and organizations should comply with. he said.

Home care services are also offered to children and families.

Home care service is not only for the elderly; Stating that it is a service that aims to provide protective, therapeutic and rehabilitative services to children and families with physical, social and emotional needs, Kumru said:

“In studies conducted in our country, the number of doctor visits for home care in pediatric patients was determined to be between one and eight times a year for a patient. The most common symptom leading to a doctor visit was fever, and routine examination was the second.

The number of visits to non-doctor health personnel in the home care service was determined to be 10 times a year, on average, for each patient. It is mostly done for routine control of children. The scope of routine control includes monitoring the physical development of the patients, performing physical therapy exercises, tracheostomy and PEG care, decubitus care, family education, checking the physical conditions including the temperature measurement of the room where the patient is located, supply of consumables, issuance of reports, drug supply and delivery, It also includes very important services such as taking blood at home and getting tests done.”

Kumru stated that the Home Care Coordination Center established by the Ministry of Health has decided whether the home care service will be given to the patient by the family doctor or by the unit working within a health institution since the patient's application to the center.

Lecturer Büşra Ecem Kumru said, “Children with special health care needs often have intellectual and physical disabilities and sensory deficits that require specific therapeutic and educational interventions. For this reason, thanks to home care, repeated and prolonged hospitalizations are reduced for these patients, and it is ensured that they receive comprehensive, inexpensive and family-centered health care in the home environment. said.

Health care in the home environment is important

Noting that following the patient in a successful and coordinated plan since the patient's successful discharge home is necessary to meet the current and ongoing health needs of the patient, Kumru concluded his words as follows:

“The aim should be to receive appropriate medical support for the healthy development of every child, to meet the needs of the patient completely in the home environment, and to reduce repeated hospitalizations in this way. The ability to cope with chronic diseases requires professional help and extends to time outside the hospital. The absence of home care services can result in damage to the health of the entire family. It is obvious that one of the most important parts of health service delivery is to provide health services in the home environment.
